literal mental illness - it's forever chicken/egg but there's no denying that trans people are less likely to be well-off mentally. I say this with sympathy and no condescention; anyway, mental illness is just a mismatch between one's mind and the society they're expected to be a part of, and in this case the key element of society they struggle with (gender identity) is pretty fucking wretched on society's part, so they're kind of right in their failure to go with that particular flow
